Description
· Implement an physical therapy rehabilitation program consistent with the physician's Plan of Treatment for our patients
· Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and Plan of Treatment.
· As needed, Oversee, supervise, educate and evaluate Physical Therapy Assistants performance in implementing physical therapy services.
· Submit timely and accurate documentation for billing purposes
· Provide hands-on care, case management and evaluation of the care plan, and education of the patient utilizing Santa Rita Home Health Software.
· Adhere to the policies and procedures of Santa Rita Home Health
· Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others
· Maintain licenses/certification and memberships in applicable professional organizations
· Coordinate treatment with other disciplines
· Assist with discharge planning by consulting with physicians, nurses, social workers, and other health care workers;
· Protects patients and employees by adhering to infection-control policies and protocols.
· Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
· Complies with all federal, state, and local Home Health professional requirements
Requirements
Four Year Degree from an approved program
Experience working in Home Health preferred
Education/experience in working with adult and geriatric population
Current and unrestricted Physical Therapy licensure
Good organizational and communication skills
· Evidence of freedom from infectious tuberculosis
· Comply with the current CDC guidance on COVID-19 vaccine, which can include vaccine booster.
· Valid Arizona driver’s license that will pass background check
· Valid Arizona fingerprint clearance card
